When you are wearing a suit, just for casual every day would a turtle neck be an okay sub or would it look dumb?
Don't wear it on your head.Some sweaters, and particularly turnlenecks, can give me a really bad headache after wearing it all day, due, I believe, to a lack of circulation of the air trapped between it and my skin. So I always wear a t-shirt underneath, which solves the problem.

As some have already pointed out, if nothing is worn under the sweater it will need to be washed after every wearing. For a fabric like cashmere that requires dry cleaning, this would be undesirable.I don't wear anything under a turtleneck, why would I?
I believe the lanolin in merino and some other wools is antibacterial which thus helps control the negative aspects of wearing such a sweater more than once w/o a washing, good for those who dislike wearing something under their sweater.
