If you want white linen, these are the best (at the price) I have found:

Note, I am not referring to the pre-folded variety. They are an abomination.

That is a good deal depending on the quality.

It's hard to beat the Brooks Brothes linen handkerchief 3-pack, although at $40 it's a bit steep up front. They'll be useful for years, though.

I think either 16x16 or 17x17. Good linen. I purchased 4 or 5 in one order a couple of years back. I wish they did different colors. That would be amazing.

How much is shipping? I can't find it on the site and it won't give me a total until I put in a credit card number.

 

I was in BB on Fifth Ave today for the final day of their 25% sale, I bought 4 for $40 before the discount. They also offer a 'bakers half dozen' for $20 if you like 100% cotton instead.

 

I prefer vintage linen squares, especially those with drawn hems.

I think if you have easy access to high-end outlet stores (i.e. Last Call, Nordstrom Rack, etc), I think that's the first place to go. I got my Talbott handrolled white linen for $5 at a Nordstrom Rack. Maybe I was lucky, but I wouldn't be surprised if I saw them at places from time to time.
