Trying to find some dress shirts in 14.5 neck and 34/35 sleeves. Not too easy. Unfortunately, I must be a pencil neck, lol.

Any good places to find an assortment of shirts in that size? If not, is it a big deal to have alterations done to take out a size 15 neck?

Altering the neck size is not a good idea. It requires replacing the collar and it will be costly and will be accompanied with the potential for other adjustment needs.
Having the shirts made special order is your best way to have very broad selectivity and the size you want.

I confirm that it is a pain to find shirts that work OTR in small neck/long sleeve combinations: I'm the same size as yours!

Years ago there was Savile Row Company offering 14.5 in various sleeve lengths, up to 35.5''. They were great shirts and I basically bought only from them. Sadly, they seem to have phased out longer sleeve lengths for 14.5 neck.

TM Lewin in Classic fit has 33.5'' for the 14.5'' but works for me since shoulders are wider and hang lower. Not the best in terms of fit but works somewhat. However, the cut is really tent-like!

I think the best solution for us pencil-neck/gorilla-arm guys is still MTM. I have already had excellent results with Savile Row Company's own MTM line.

@ratioclothing, which uses the same American factory as Brooks Brothers at about half the cost (or a New Jersey factory now demand has increased). Not only could they address size though also your body shape (had mine adjusted for strait shoulders and fit wonderfully) being they craft custom shirts (between MTM and bespoke). And of course amazing customer service.
