I would measure a couple of places to figure out the effective chest (which won't be exactly 46, spoiler alert).

First across the back two panels, right at the pit/bottom of armhole. Then the side panels. Then try and estimate the front panels, keeping in mind that the lapel is going to make it more difficult.

Measuring across the front lapels (as in your picture) includes the gap between them (which will likely make it difficult to be precise) and doesn't necessarily include the side panels, or even half of them.

Chest measurement is difficult to do well. Most of us would agree that if your jacket is laid out properly and measures 21 inches it likely is not a 46R. But first real test is does it fit you. Second is what size does it say it is. If a good garment it will have the size inside the left or right inner pocket. Third is put it on and have someone measure you under the arms with the jacket on.
