Dress shirt question- Sleeve buttons mandatory?

Curious as to how folks feel about a button on the shirt sleeve? Is it inelegant not to have a button on the sleeve? More particularly is it acceptable with button cuffs. I rarely see shirts without the sleeve button but i think its kind of neat looking (dont know what the exact button is is called) ...
